I've gotten the form on line but it says I need original documents. Is it easier to go in person and if so, where can I go? I believe a copy of the birth certificate is required. It was painless. This website can help you fid the closest office: http://www.ssa.gov/ I know someone who just got a replacement card through a NY SS office and it arrived in about 3 weeks.
