Does anyone know how to get a replacement card for a minor. I've gotten the form on line but it says I need original documents. Is it easier to go in person and if so, where can I go? Thanks in advance.

I think you have to contact the Dept. of Records (or whatever it's called) in the town where your child was born. I believe a copy of the birth certificate is required.

Since you obviously have the SSN, it shouldn't be too hard to get, one would think.

I had to get a replacemet card for my second child. It was painless. Send the original documents and they send them back with the SS card.

They mail you the replacement card whether you take the original documents to the office or mail the docs.

This website can help you fid the closest office:

I know someone who just got a replacement card through a NY SS office and it arrived in about 3 weeks.
